Who qualifies for free ECM services?
You must be an active Medi-Cal beneficiary enrolled in Molina, LA Care, or Health Net, and live in Los Angeles, Riverside, or San Bernardino County. Additional qualifying factors include chronic health conditions, recent hospitalizations, or limited social support.
What if I don't qualify for free services?

Do you provide services in my home?
No, all our services are provided outside the home. We transport you to healthcare facilities, government offices, and community resources, providing accompaniment, translation, and coordination services at those locations, then safely return you home.
Which managed care plans do you work with?
We work with Molina Healthcare, LA Care Health Plan, and Health Net. You must be enrolled in one of these plans to qualify for free services.
Transportation and Appointments
How does transportation coordination work?
We provide door-to-door transportation for medical appointments, pharmacy visits, and essential services. Our team picks you up from your home, coordinates pick-up times, ensures you arrive safely and on time, and safely returns you home after your appointment.
Can you help with translation during medical visits?
Yes, we provide real-time translation and interpretation services during appointments at healthcare facilities, ensuring clear communication with doctors, nurses, medical staff, and administrative personnel. We accompany you into appointments and provide language support on-site.
How far in advance should I schedule services?
We recommend scheduling routine appointments at least 48-72 hours in advance. For urgent medical needs, we can typically accommodate same-day requests.
Will you accompany me into my medical appointments?
Yes, our community health workers transport you to healthcare facilities and can accompany you into appointments, providing translation services, helping with paperwork, and ensuring you understand medical information and instructions while at the healthcare facility.
Benefits and Housing Assistance
What types of benefits do you help with?
We assist with SSI (Supplemental Security Income), EBT (Electronic Benefit Transfer), housing applications, Medi-Cal renewals, and other government assistance programs. We transport you to government offices and accompany you on-site to complete application processes.
Can you help me find senior housing?
Yes, we provide practical support for finding, applying for, and securing affordable senior housing options within our service areas. We transport you to housing offices and property visits, accompanying you through the application process on-site.
How long do benefit application processes take?
Processing times vary by program. SSI applications can take 3-6 months, while EBT and housing applications may process faster. We keep you updated throughout the process.
Family and Caregiver Support
Do you help family caregivers?
Yes, we provide support and resources for adult children or relatives managing family member healthcare needs. This includes medical appointment coordination, transportation arrangements, and connecting families with other resources. We can transport your family member to appointments and provide on-site support.
Can multiple family members receive services?
Each person must meet eligibility requirements individually. However, we can coordinate services for multiple eligible family members.
